Blepharoplasty

Blepharoplasty (eyelid surgery) represents one of the most delicate cosmetic and functional procedures involving the face. Designed to restore a more youthful appearance in an area that conveys so many emotions, subtle changes, when executed correctly, can be transformative. Upper eyelid

Lower Eyelid

In the lower eyelid, the procedure involves transferring the prolapsing fat pads across the bony orbital rim through hidden incisions (transconjuctival approach). This approach can have a powerful rejuvenative effect as it redistributes volume, smoothens the contours of the region and improves aging hollowness. The traditional approach that most surgeons adopt (transcutaneous incision) is a subtractive form of surgery that can lead to functional and aesthetic complications such as eyelid malposition, dry eye and rounded eyelid corner.

ASIAN BLEPHAROPLASTY

How are Asian eyelids different from Caucasian eyelids?

Asian eyelids are unique as the eyelid crease is quite different from the Caucasians’ in terms of length, shape and height. Although about half of the Asian population does have a fold in upper eyelid area, the other half does not. The former type is traditionally called the “double eyelid” type where the actual eyelid height is higher, exposing more of the eye.

The difference is anatomical and relies on the degree of the attachments of the levator muscle on the eyelid skin. In Asians the levator muscle inserts either very low (with partial or low creases) or not at all so that no crease is visible. In addition the weak insertion of the muscle allows the eyelid fat to be placed at a lower position, making the eyelid to appear fuller and the eyelid opening narrower.

While facial attractiveness does not necessarily relate to the eyelid shape and fold, many Asians desire the double eyelid crease, as it gives an overall impression of a larger appearing eyelid opening. Known as “Asian double eyelid surgery” a blepharoplasty procedure can be performed to create a natural-looking crease. The goal of double eyelid surgery is not to westernize an Asian face, but to create a better defined crease and enhance the overall appearance of the upper face.

What type of procedures are used to treat Asian eyelids?

Two types of procedures are available to rejuvenate Asian eyelids: partial incision and incisional surgery. The partial incision which relies on the use of sutures is minimally invasive and potentially reversible but the permanence of this procedure is questioned and it cannot address anatomic aspects like prominent fat or epicanthal folds. This procedure is best suited for younger patients where the upper eyelid skin is relatively thin, with thin orbicularis muscle and eyelid fat.

The incisional surgery is a delicate but powerful procedure where the incision line is hidden in the newly created eyelid crease. The technical demands and details of this procedure rely on the design of the new crease as well the technical skills required for the incision to heal in a permanent but natural fashion. For a natural looking fold, the ideal amount of pretarsal show is 2-3 mm and this height is usually achieved by creating an incision at 7-10 mm above the lash line at the mid pupil. A small amount of fat and muscle is removed to allow stronger adhesion of the skin to the deeper tissues. A series of sutures are placed through the levator muscle to facilitate this creation.

What other aspects must be taken into consideration with Asian Blepharoplasties?

A series of additional aspects must be considered when performing double eyelid surgery like facial asymmetry, the position of the eyebrow, as well as the epicanthal fold, a web like skin fold that drapes over the inner corner of the eye. This fold can be a non-desirable ethnic trait as a wide fold can make the nose to appear even flatter further exacerbating the lack of definition of the nasal bridge usually encountered in Asian patients.

Although both treatments have the same objective in enhancing the definition of the upper eyelids, the Asian blepharoplasty primarily aims in creating a fold or crease in the upper eyelid that runs more parallel to the eyelid margin and blends with the normal skin( epicanthal) fold in the inner corner of the eye. The goal of Asian eyelid surgery is not to westernize an Asian face, but to create a better defined crease and enhance the overall appearance of the upper face.

How long does the procedure last and what is the anesthesia used for this procedure?

Although the duration of surgery can vary from patient to patient, typically it takes an hour and a half to perform eyelid surgery. Patients can return to their daily activities and working schedule within one week. The procedure is usually performed under local anaesthesia with mild sedation. With this approach patients experience fewer side effects and typically can be discharged home safer and quicker within a few hours following surgery.

What are the possible side effects related to Asian blepharoplasty?

As with any surgical intervention the procedure can carry some risks which are extremely rare and are usually minimized through careful pre-operative screening of the patient and choosing a fully qualified experienced plastic or oculoplastic surgeon.

Some patients can experience minor side effects like temporary eye irritation and blurriness, swelling, asymmetry of the eyelids and changes in tearing. These particular effects are usually temporary and settle in the first weeks after the operation, as the tissues recover from the procedure. Infection and bleeding are minimized through careful technique and antibiotic cover.

Eyelid Lift

The upper eyelid levator muscle complex is the muscle responsible for the eyelid’s position and dynamic excursions. Ptosis is defined as abnormal lengthening of the levator muscle causing lowering of the upper eyelid margin. Ptosis not only affects the aesthetic appearance of an expressive part of the face but can also compromise a patient’s field of vision.

Ptosis can also lead to secondary compensatory effects like overaction of the frontalis muscle in the forehead with eyebrow arching and asymmetry. In congenital cases, children with ptosis may develop amblyopia (lazy eye) or even developmental delay from limitation of their vision.

Surgical correction of ptosis enables reacquisition of the levator muscle tone and appropriate repositioning of the eyelid margin. This intricate surgical process involves balancing delicate relationships with the position of the contralateral eyelid and the eyebrow compensatory arching. Anatomic considerations of bony structure and individual soft tissue distribution are equally important to optimize surgical outcomes.

In the majority of cases the procedure is performed through the undersurface of the eyelid and without any incision made on the eyelid skin. This approach named Mullerectomy involves strengthening the levator muscle complex through shortening an underlying accessory muscle (Muller muscle) and conjunctiva. This scarless technique, which can be performed under local anesthesia alone, has the advantage of predictability and can raise the eyelid margin up to 3 mm. By balancing the amount of Muller muscle resection, and hence the degree of levator muscle reinforcement, subtle refinement and tuning of eyelid position can be achieved.

For patients with significant stretch of the levator muscle, an anterior surgical approach through a hidden skin crease incision might be necessary. This process involves a more complex manipulation of the eyelid soft tissues and requires patient cooperation as the muscle and eyelid area cannot be entirely anesthetized during surgery.

In congenital cases where the levator muscle is inherently abnormal and weak, a suspension sling is inserted to link the eyelid and eyebrow soft tissues. This ingenious procedure recruits the frontalis muscle to achieve the lifting effect. The frontalis is the main muscle of the forehead and its main function is eyebrow elevation. This minimally invasive procedure, which involves access through well- hidden entry points, has to be performed before the vision obstruction impacts the visual development of the child.

Eyebrow Life

Endoscopic reshaping of the upper face

The endoscopic eyebrow lift is a minimally invasive rejuvenating procedure that counterbalances sagging and volume redistribution in the ageing eyebrow. The endoscopic lift is customized to suit individual aging features such as soft tissue deflation, ligament tension, bony contours and muscle interplay.

The procedure aims to revitalize the outer third of the eyebrow and is performed with the use of a telescopic devise-camera, inserted via three small incisions hidden within the forehead hairline. Facial Palsy

Overcoming the effects of facial paralysis is a long process requiring multidisciplinary input, subspecialty expertise and a staged systematic approach. Whether the journey of facial reanimation requires retraining of muscles and nerves or surgery, the aim is to improve symmetry of the face and protect the vitality of the eye surface.

One of the key treatment objectives is to restore the dynamics of the eyebrow-eyelid complex and orbicularis oculi muscle. The orbicularis is a key muscle of the periocular area, responsible for the resting tone and closure of the eyelid. The intricate balance between the action of the orbicularis (a protractor muscle that closes the eyelid) and the levator palpebrae muscle (a retractor muscle that antagonizes the orbicularis by lifting the upper eyelid) is affected by facial palsy at the advantage of the eyelid retractors. As a result impairment of eyelid closure occurs and regular lubrication is essential to maximize the protective barriers of the corneal surface.

Depending on the underlying aetiology and the degree of anticipated neuromuscular recovery, surgical rehabilitation of the periorbital area aims to reinforce the dynamic excursion and closure of the upper and the positional tone of the lower eyelid. Additional interventions can address eyebrow and lower face asymmetries.

The denervation of the orbicularis oculi muscle, can be counterbalanced by weakening the action of its natural antagonist muscles. In the upper eyelid, weakening the levator palpebrae superioris muscle (the muscle that primarily opens the eye) can be achieved nonsurgically or surgically. Botulinum toxins may be selectively injected into this muscle to induce relaxation of the eyelid and thus improve eyelid closure. Surgical weakening of the levator muscle can be performed via a hidden approach from the undersurface of the eyelid: a procedure named transconjuctival recession. Alternatively, placement of a gold or platinum weight through a small incision within the eyelid crease can improve the dynamic excursion of the upper eyelid and protection of the corneal surface.

Additional reinforcement of the tone of the lower eyelid by strengthening the ligaments at the corner of the eye (lateral canthal resuspension or canthoplasty) can improve the position the position and the interface with the ocular surface. Volumizing agents like hyaluronic acid gel injections may play an important role in the early stages when surgery is not recommended. Placement of the filler in the upper eyelid and within the plane of the levator muscle provides a “gold weight” effect and counterbalances the retraction of the upper eyelid. For patients with transient facial palsy with good prognosis for recovery of facial muscle function, hyaluronic acid gel injection is an elegant way to protect the eye and avoid surgery.

In similar fashion, injections of hyaluronic acid gel within the lower eyelid complex can improve the position of the eyelid margin, ameliorate asymmetry, and most importantly minimize exposure of the corneal surface and lagophthalmos. Botulinum toxin type A (BTTA) injections may have an important role in balancing the action of overacting muscles and symptoms of synkinesis. Synkinesis is the result of abnormal facial nerve regeneration and manifests as abnormal synchronization of facial movements, where muscles other than those intended contract together during specific actions of the face. The most common effect of synkinesis is when patients experience eye closure or twitching while smiling or chewing.

BTTA injections can relax the unwanted muscle movements on the normal side of the face and reduce tension in areas of the face that are hyperactive due to synkinesis. Treatment of synkinesis in conjunction with re-education of muscles and nerves can be initiated at any time after symptom onset and augment results of medical and surgical therapies.

Face Balancing

Devised by Dr Papageorgiou this facial remodelling procedure works to restore facial symmetry and youthful contours by combining hyaluronic acid gel fillers and wrinkle smoothing injections in a single session. Jaw Remodelling

While a strong, square jawline can be an attractive facial feature for men, for women a softer and more triangular shape is often more desirable. Masseter hypertrophy is a clinical phenomenon characterized by a soft swelling near the angle of the mandible that changes the feminine features of the lower face.

Bone structure varies in ethnicities; the lower third of Asian faces is typically wider than that of Caucasians and is determined by the size and width of the mandibular bone and the thickness of muscles and subcutaneous fat surrounding it. However, masseter hypertrophy can appear in other ethnicities as personal habits can be directly responsible for the enlargement of the masseter: excessive gum chewing, chewing hard items or stress and joint misalignment can lead to bruxism (grinding the teeth at night). The swelling may be associated with facial pain and can be prominent enough to be considered aesthetically unattractive.

Changing the shape of the lower face from square to a more triangular one has become an increasingly popular procedure, particularly for Asian females, and Botulinum toxin type A (BTTA) has significantly advanced our ability to create an aesthetically slim, smooth and more feminine jaw contour line without surgical intervention. Reducing the thickness of the masseter also contributes to the projection of the cheekbone, which further enhances the ‘heart shaped’ face.

BTTA is a powerful neurotoxin produced by the anaerobic organism Clostridium botulinum. When injected into a muscle it causes interference with the neurotransmitter mechanism by producing selective weakening and subsequent atrophy of the muscle. The toxin selectively remodels the masseter over time, leading to an overall reduction in size and projection of the jaw. Careful analysis of the facial contours and anatomy of the facial muscles, in addition to skilful and precise placement of the neurotoxin are critical in achieving aesthetically pleasing results. Patients typically notice softening of the masseter muscle at two weeks and reshaping of the jawline at six weeks. With two injections spaced three months apart it is reasonable for patients to expect long-term benefits however it will not yield permanent changes.

Profhilo Hydralift

Experience a new method of facial remodelling via a superficial intradermal injection of a complex of high- and low-molecular-weight hyaluronic acid. Two sessions can counteract volume loss in the skin, restoring hydration, elasticity and skin tone. EMFace

EMface is the latest innovation in energy based devices that utilizes the simultaneous application of High Intensity Focused Electrical Muscle Stimulation (HIFESM) and Synchronised Radiofrequency (RF) to strengthen a selective group of facial muscles, aiming to improve their lifting action on the face.
